SERVICE AGREEMENT
Service Agreement | Marion's Senior Errand & Non-Medical Support Service
This Service Agreement ("Agreement") is entered into between Marion's Senior Errand & Non-Medical Support Service ("Company") and the client ("Client"). By requesting or using services, the Client agrees to the following terms and conditions.
1. NATURE OF SERVICES
Marion's Senior Errand and Non-Medical Support Service provides non-medical support services including, but not limited to:
- Prescription pickup
- Grocery shopping
- Post office errands
- Transportation for non-medical purposes
- General companionship and assistance
Disclaimer: Marion's Senior Errand and Non-Medical Support Service does NOT provide medical care, nursing services, diagnosis, or treatment. Services are strictly non-medical in nature.
2. CLIENT RESPONSIBILITIES
The Client agrees to:
- Provide accurate and complete information
- Ensure a safe environment for service delivery
- Provide clear instructions for errands
- Be available for communication when necessary
Marion's Senior Errand and Non-Medical Support Service is not responsible for delays or issues caused by incomplete or inaccurate information.
3. AUTHORIZATION FOR ERRANDS
The Client authorizes Marion's Senior Errand and Non-Medical Support Service to perform errands on their behalf, including prescription pickup where permitted by law. The Client agrees to provide any required identification, payment, or documentation necessary to complete such errands.
4. PAYMENT TERMS
- Standard Rate: $25 per errand service (unless otherwise stated)
- Payment is due at the time of service or prior to service completion
- Additional fees may apply for extended wait times, long-distance travel, or special requests
Accepted payment methods will be communicated at booking.
5. CANCELLATION & REFUND POLICY
- Cancellations must be made at least 24 hours in advance
- Same-day cancellations may be subject to a fee
- No-shows may be charged the full service amount
- Refunds are issued at the Company’s discretion based on service circumstances
6. LIMITATION OF LIABILITY
Marion's Senior Errand and Non-Medical Support Service shall not be held liable for:
- Delays beyond its control (traffic, store closures, etc.)
- Errors due to incorrect client-provided information
- Third-party issues (pharmacies, stores, etc.)
Liability, if any, shall be limited to the cost of the service provided.
7. NON-MEDICAL ACKNOWLEDGMENT
The Client acknowledges that Marion's Senior Errand and Non-Medical Support Service is not a licensed healthcare provider and does not offer medical advice or services.
8. PRIVACY & CONFIDENTIALITY
Marion's Senior Errand and Non-Medical Support Service respects client privacy and will only collect necessary information to perform services. Personal information will not be shared with third parties except as required to complete requested services.
Marion's Senior Errand and Non-Medical Support Service does not store or manage protected health information beyond what is necessary for service completion.
9. SAFETY & RIGHT TO REFUSE SERVICE
Marion's Senior Errand and Non-Medical Support Service reserves the right to refuse or terminate services if:
- The environment is unsafe
- The Client or others exhibit inappropriate or threatening behavior
- Requests fall outside the scope of non-medical services
10. DISPUTE RESOLUTION
In the event of any dispute, both parties agree to first attempt to resolve the matter through informal discussion. If unresolved, the dispute shall proceed to mediation in the State of Alabama. If mediation fails, the matter shall be resolved through binding arbitration in Lee County, Alabama.
This Agreement shall be governed by the laws of the State of Alabama.
11. ENTIRE AGREEMENT
This Agreement constitutes the entire understanding between the Client and Marion's Senior Errand and Non-Medical Support Service and supersedes all prior discussions or agreements.
12. ACCEPTANCE OF TERMS
By booking or using services, the Client acknowledges that they have read, understood, and agree to the terms of this Agreement.
